To subscribe to the Calmuni Subscription Service, a Subscription Contract must be completed and returned to us. You may use one of two options:

1. View the subscription contract with subscription rates, print it, sign it and either fax it or mail it back to us.
2. Send us a fax at (510) 658-2642 or email message and we will gladly send you a contract and any additional information that you desire.
